About His Hands

His Hands was born out of a passion and a vision to the hands and feet of God in the earth. We can only touch God as we touch others. I believe that everyone wants to be whole, physically, spiritually and emotionally. When we come alongside one another (for real) to educate, train, support and advocate for each other, we become instruments of healing and tools of empowerment for that purpose.

His Hands at Grace Community Village is a first step in the vision to alleviate barriers to healthcare for every marginalized citizen of this city.

Our goal is to make healthcare disparity in our community a thing of the past. We are going to change the statistics that say:

1. HIV infection is fastest growing among African American women and youth.

2. African Americans, Hispanics and the poor die of complications related to diabetes, heart disease, cancer and stroke more than any other ethnic groups.

3.African American babies have lower birth weights and more incidences of infant death that other ethnic populations.

We can make a difference; one person, one group, one block, one neighborhood, one sector and one city at a time. It will take time, but have no doubt, make a difference we will!

His Hands Community Nursing Center at Grace Community Village offers various services to help you maintain wellness and manage your healthcare including information and answers to your questions.

Health Education
a. What is sugar? High blood pressure?
If my mother had it, will I?
b. What are your risk factors? What can you change?
How can you change?

Disease Management
a. How do I test my sugar? My blood pressure?
b. What does the medicine do? Does it matter when I take it?

Wellness and Prevention
a. Sugar testing and blood pressure checks
b. Exercise classes and groups
c. Classes and presentations about different ways to stay healthy
d. Cooking your food in a healthy way that still tastes good

Support
a. Peer group meetings
b. Counseling and Spirituality groups
c. Referrals to other sources of help and information

Mental Health
a. What is mental health? Does it mean I am crazy? How do I know if I have a mental health problem? Who can tell?
b. Referrals to people and places that can help

Community Outreach
a. Giving health and wellness presentations, classes or
training at your building or organization
b. Screenings at community events and places

Consumer Advocacy
a. Attending doctor appointment at your request
b. Follow up on your concerns about your care, at your request
c. Hearing what you say!
d. Teaching you how to work through and work with the healthcare system

These are some of the services we will be providing to help you help yourself to be healthy, whole and IN CHARGE of your health.

Community Collaboration

1. Join other organizations to help get your message out
2. Provide space for you to give presentations or classes
3. Nursing, social work, human services and medical professionals - have students come work and learn at our center
